If you make a mistake you can use the "Reset Form" button to clear your entries and start over.

Use the "Submit" button to send the form to Department. You will receive a confirmation email that lists the information that you entered. To correct an error after submitting the form simply submit a new report using the same form and selecting "Corrected" and the Submission ID in the "Corrected Session ID" field which you will find listed as Submission ID in the email that you received.
