Instead, please use this form to submit to the Department each of your Delegates elected under Section 222 of the National Bylaws and Manual of Procedure. Required fields are annotated with an asterisk (*). When you submit the form, you will receive an email showing the information that you entered. Please forward that message to the delegate.
